I agree to commit to 50 candle sales during my month. In the event that I do not sell all candles, I understand that I will have one extra month to sell my remaining candles. I agree to make the difference in a donation to W.I.N. with any unsold candles and that donation will be due within 30 days of the last day of candle sales. I agree to put my credit card on file.

I understand that these candles will be available only through the WIN and Mrs. Globe website. I understand that if I choose I can pre-order my candles and sell them directly with the proceeds going to W.I.N.

I understand that the Mrs. Globe and W.I.N. Foundation will also be promoting my candle during my featured month.

I understand that as a candle sponsor, I will be a featured guest at the W.I.N. Celebrity Luncheon where I will have a table dedicated to showcase my candle .

I have filled out the above form completely and to the best of my ability and understand that if I have NOT been approved to create a custom candle, the form will be invalid.
